If you ever get kicked out of a map and or disconnected and when you login you're surround by 20 or 30 other people in the maps entrance then that's them. The instance goes down, but a different machine just opens a new instance and you reconnect. Or it goes down completely and you can't login again for a length of time.

Just wanted to note that this likely isn't an issue with Guild Wars 2 itself, but a service sitting in front of the game (and all ANet services).
